A non-union occurs when a bone fails to heal within a normal timeframe. This can result from poor blood flow, stability issues, or infection. Types & Treatments

Non-unions are categorized and treated based on bone vascularity:

  • Hypertrophic Non-union: Adequate blood flow but inadequate mechanical stability. Treated with heavier compression plates or dynamic nails.
  • Atrophic Non-union: Poor blood supply and biological failure. Treated with autologous bone grafts and biological stimulants.
  • Infected Non-union: Managing deep localized infection using antibiotic bone cement beads prior to revision hardware.

Diagnostic Checklist

- Persistent Loading Pain

Dull pain and instability at the fracture site when putting weight on the limb months after injury.

- Serial X-rays

Checking for a persistent gap or smooth bone edges (pseudarthrosis) over consecutive monthly scans.

- Blood Panels

Ruling out calcium deficiencies, Vitamin D3 depletion, or hidden low-grade bone infections.
